How We Are Different: Value-Based Care

We listen. We see you. We take the time during your visit to understand where you’ve been, what is working (and not working), and where you’d like to go. A comprehensive plan that includes lifestyle change takes time and thoughtfulness. Education is key. Your input is essential. This type of care is often not possible in the conventional medical practice setting, but we hope more practitioners can provide this in the future.

What conditions or issues do you treat? 

As Family Medicine providers, we are accustomed to evaluating the whole person, both for prevention and risk assessment appropriate to your age and stage, as well as individual body system issues. We commonly help patients with gut health, hormonal health, autoimmune disease, cardiovascular and metabolic health, mental and emotional health, as well as musculoskeletal and dermatologic issues. We utilize specialists when needed, and have grown a caring and capable village to support your needs. 
Many times we act as “Quarterback” to coordinate your care among multiple specialists. We can act as your primary care provider, or you can retain your current PCP and come to us for consultative care for a specific issue or an Integrative approach.

How To Become a New Patient - Our Process

1. Fill out a New Patient Application
Your responses to this application help our team understand your health goals, expectations, and the type of support you’re seeking. A member of our care team will follow up with you either way — whether you are accepted as a new patient or not — so you never have to wonder about your status.
2. Initial Visit 
Your initial visit is a 2-hour comprehensive intake appointment in which we review your medical records, take a thorough history, perform a physical exam (as indicated), and begin to develop a care plan to address your concerns and put you on the path to well-being. This visit includes body composition analysis via our InBody machine . 
3.  Subsequent Visits and Services
  • Follow-Up Visits - Available in person or via Telemedicine (Video or phone); typically a 30 or 60-minute appointment
  • Acute Visits - Available to established patients for acute need or illness
  • Nurse Visits - Our nursing staff is attentive to your needs for coordination of care
  • Phlebotomy (Lab Draw) Services - We have partnered with Estelle Powell, our excellent phlebotomist, to provide lab draw services in the office one day a week. Her business also is able to do mobile lab draw for an added cost.
